You should have received Form 1099-R in the mail. If you haven’t, you can view it through your ERS OnLine account.

To view or print your form in ERS OnLine:

1. Look for "My Payroll Information" on the Retiree homepage
2. Click “1099-R Summary.”

You can also call (866) 399-6908 (TTY: 711) to ask for another copy to be mailed to you.

What is Form 1099-R?

Form 1099-R shows distributions (gross earnings) from  your annuity and the taxes that were withheld. You can find additional information on ERS’ Form 1099-R FAQs webpage.
