Continue Coverage

When can you enroll in COBRA?
Enroll when your employment ends or a dependent loses coverage.
Former COBRA Unmarried Child (FCUC)
Unmarried dependent over age 26 can continue health coverage under COBRA.
Extending COBRA coverage
Cover may be extended to 36 months if during the coverage period you experience a secondary qualifying event.
Automatic Withdrawal/Cancellation of Insurance Premiums
Form for direct pay retiree, surviving spouse, and COBRA participant to authorize direct withdrawal of monthly insurance premiums from bank accounts.

Using network participating dentist:

Deductible - $0 for preventative services

Copays - Vary by services; $0 for preventative

Cleanings/Exams - Allows 2 per calendar year at no cost

Orthodontic - $1,500 max benefit (age 19 and younger)
HumanaDental DHMO Coverage
Must choose a network dentist to receive covered benefits.

Deductible - $0

Copays - Vary by services

Cleanings/Exams - Allows 2 per calendar year with $12 copay

Orthodontic - Services performed by a general dentist listed in the directory – child - $1,800, adult - $2,100; services performed by specialist – You pay 75% of his/her usual fee. DHMO pays nothing.
