Optional Term Life Policy Claim

Your death must be reported to ERS to obtain the instructions and forms required to file a claim for benefits. The forms are sent to the designated beneficiary. Benefits are paid at the amount of coverage you selected unless you have taken Accelerated Life or Viatical Benefit. You beneficiary receives the reduced amount if you have taken these benefits.

ERS will need only one death certificate to access all benefits.
