How do I file a claim?

If you are certified as totally disabled by your physician:

1. Download a Disability Claim Form from the Dearborn website.
2. Complete all identifying information in the Employee’s section of the claim form. 
3. Ask your benefits coordinator to complete the Employer’s Section, attach all required documentation, and return to you.
4. Have your doctor complete the Attending Practitioner’s Statement as proof of your disability.
5. Complete all information and mail the claim form and all documentation using the address at the top of the form.
