Not eligible for Voluntary AD&D coverage

The following persons are not eligible:

• If your spouse is already insured as an employee under Employee Only Voluntary AD&D, he or she cannot receive coverage as your dependent as well.
• Any person who is currently in the process of converting Basic or Optional Term Life insurance to an individual policy of whole life insurance.
• Retirees and former employees.
