EOI Application Process
Proof of good health, also called evidence of insurability, is required for some types of coverage. Evidence of insurability (EOI) is an application process in which you provide information on the condition of your health or your dependent's health in order to be approved for coverage.
Follow these steps to apply for life insurance through Minnesota Life (third-party administrator):
- Fill out the application – Include your state agency or higher ed number and employee ID number assigned by ERS. Sign into your account to find your employee ID number (located under Account Balance Summary or Beneficiary Summary).
- Send in your application to Minnesota Life – Make sure you didn’t leave any blanks. Missing information will slow down the processing of your application. Providing false answers on an EOI application for any GBP coverage is a violation of state insurance laws. Send in as early as possible to the address shown on the EOI application.

- Fill out the application – Include your state agency or higher ed number and employee ID number assigned by ERS. Sign into your account to find your employee ID number (located under Account Balance Summary or Beneficiary Summary).
- Send in your application to Dearborn National – Make sure you didn’t leave any blanks. Missing information will slow down the processing of your application. Providing false answers on an EOI application for any GBP coverage is a violation of state insurance laws. Send in as early as possible to the address shown on the EOI application.
During the application process, you may need to provide medical records, update medical information, receive a paramedical exam or complete a questionnaire.
After Review
After your application has been reviewed, you will receive an approval or denial letter.
If you are an active employee, advise your benefits coordinator or accessHR to enter the election on your behalf once you receive your approval letter. Your coverage starts the first of the month after receipt of the approval letter.
If you are a retiree, ERS will set up your coverage to begin the first of the month after ERS receives the approval letter.
