Voluntary ADD

  • This benefit provides additional protection for you and your family in the event of accidental death or certain accidental injuries. Coverage is purchased in increments of $5,000 with benefits between $10,000 and $200,000. This is in addition to AD&D benefits provided with Basic and Optional Term Life insurance for active employees.

    You can choose from two levels of coverage: you only or you and family. You cannot convert this coverage to an individual policy.

    Minnesota Life is the insurer for Voluntary AD&D insurance.

    Coverage is purchased in increments of $5,000:

    • the minimum coverage available is $10,000, and
    • the maximum coverage available is $200,000.

    Coverage amounts begin to decrease when you reach age 70.

    Accidental death benefit

    In the event you die as the direct result of an accidental bodily injury, your beneficiary will receive the amount of insurance you selected when you signed up for coverage. Your death must be reported to ERS.

    Dismemberment benefit

    If you have an accident and suffer one of the covered injuries, you will need to file a Dismemberment Benefit Claims Form. If one covered bodily losses occurs, you will receive AD&D benefits of one-half of your Voluntary AD&D benefit amount. If two or more bodily losses occur at the same time, you will receive AD&D benefits equal to the Voluntary AD&D benefit amount.

    Family Coverage

    Accidental death benefit

    If a covered family member dies as a direct result of an accidental Bodily Injury, you must report the death to ERS. Your spouse’s benefit is 50% of your amount of coverage.

    Your child’s benefit is either:

    • 5% of your coverage amount, if there is a spouse who is eligible for the insurance, or
    • 10% of your coverage amount if there is no spouse who is eligible for the insurance.

    Dismemberment benefit 

    If a covered family member has an accident and suffers one of the covered injuries, you will need to file a Dismemberment Benefit Claims. If the participant suffers one of the covered bodily loss, you will receive AD&D benefits equal to one-half of the Voluntary AD&D benefit amount they are eligible for. If the participants suffers two of the covered bodily losses at the same time, you will receive AD&D benefits equal to the Voluntary AD&D benefit amount they are eligible for.
