For claims after January 1, 2012, Minnesota Life will be the underwriter for the coverages listed above.

Dearborn National will continue to be the underwriter for:

• Short-term Disability insurance,
• Long-term Disability insurance, and 
• Applications to Request Continuation of Coverage for Disabled Dependents Age 26 and Over.
