Community First Health Plans

Community First Health Plans is a Health Maintenance Organization (HMO) that serves counties in the San Antonio area. You must reside, live, or work in the service area to be eligible for coverage under Community First.

• take an online health assessment,
• check claim status, and
• communicate with member services.
